CUA offers three Delta Dental options and DentaChek. Employees enrolled in a CUA medical option receive the DeltaCare HMO or Dental Standard PPO at no additional cost.

 

Delta Care HMO	Care Provided by DeltaCare dentists. Diagnostic and preventive services are covered at little or no cost to you. Out-of-pocket costs are clearly defined. No deductibles or annual and lifetime maximums. Includes orthodontic coverage for adults and children
Delta Dental Standard PPO	Allows freedom to receive in-network or out-of-network dental care. Copays are a percent of the cost of services received. Copays are higher for care received out-of-network. Annual deductible applies to all services. No orthodontic coverage.
Delta Dental Buy –Up PPO	Allows freedom to receive in-network or out-of-network dental care. Copays are a percent of the cost of services received. Copays are higher for out-of-network care. Diagnostic and preventive care covered at 100% of Delta’s allowed amount. Orthodontic coverage for dependent children under age 19. Annual deductible applies to all services expect diagnostic, preventive and orthodontic services.
Denta-Chek	Denta-Chek discounted dental plan covers office visits, exams, cleanings and most x-rays. Participants receive fillings, crowns, dentures, braces, periodontics and endodontics at reduced fees. You may enroll at any time. You must enroll for one year of coverage.
